Patients will require blood work and a consultation prior to starting treatment. ONLY the blood work and consultation can be billed through insurance. Patient is responsible for any co-payment or additional charges. If patients do not have insurance or do not want to use their insurance there will be a $99 consultation fee. Out of pocket lab fee can be quoted upon request. Please verify your benefits with your insurance provider before your appointment-copays and deductibles may apply.
